| 1. CCHD continues to order
and receive shipments of H1N1 doses (Nasal spray, multi-dose
injectable and pre-filled syringes).
2. Vaccine continues to be distributed to pediatricians,
OB/GYN practices, primary care providers and scheduled
school clinics.
3. Free H1N1 clinics will be offered to school district
residence (see schedule below) at select schools. Students
will be immunized in the morning followed by community
members starting at noon in most cases. Community members
will be informed via reverse 911 calls if their school
district clinic has been opened to the public. Intake
forms/packets will be available at the clinics. However, for
those that would like to complete their forms before

6. Governor Paterson announced on Thursday that,
effective immediately, health care providers may make the
H1N1 flu vaccine available to all individuals who want the
vaccine, including those who are NOT in the priority groups.
7. The Chautauqua County Department of Health will be

like them at regularly scheduled Health Department
immunization clinics, this week. The following vaccination

spray and pre-filled syringes) on 11/19/09.
2. Approximately 1400 H1N1 doses were administered to
Dunkirk City School students. Due to the shortened school
week in observance of the Thanksgiving holiday, no H1N1
vaccination clinics are being held this week.
2. Vaccine continues to be distributed to pediatricians,
OB/GYN practices, primary care providers and scheduled
school clinics.
3. The Bemus Point Elementary School and Maple Grove High
School clinic dates are set for November 30th and Dec. 1st
respectively and start at 9:00am.
4. Southwestern Central District dates have been set for
Dec. 3rd and 4th.
5. Clymer School District has tentatively been set for
Dec. 8th or 9th. Chautauqua Lake School District has
tentatively been set for Dec. 9th or 10th, and Fredonia
School District has tentatively been set for Dec. 14th thru
17th. The tentative dates are subject to change based upon

|
Chautauqua County Health Department received another shipment of
H1N1 vaccine (nasal spray and pre-filled syringes) on Monday 11/16/2009.
Approximately 600 doses of H1N1 vaccine were administered to Silver
Creek School students.
This week, H1N1 vaccination clinics are being held at the Dunkirk
City Schools.
The Maple Grove School Clinic dates are set for Monday, November
30th and Tuesday, Dec. 1st at 9:00am.
Upcoming tentative dates for H1N1 vaccine clinic
school locations (all dates are subject to change based upon vaccine
availability and school readiness):
1. Southwestern School Clinic dates have been tentatively
set for Dec. 2nd and Dec. 3rd.
2. Clymer School Clinic dates are tentatively set
for Dec. 8th and Dec. 9th.
3. Chautauqua Lake School Clinic dates are tentatively
set for Dec. 9th and Dec. 10th.
4. Fredonia School Clinic dates are tentatively
set for Dec. 14th thru Dec. 17th.
There will be an H1N1 Vaccine Clinic specifically for school nurses
on Thursday, Nov. 19th from 6:00 to 8:00p.m. in the 4th floor Conference
Room at the HRC Building in Mayville.
This clinic is exclusively for full-time school nurses.

|
The Chautauqua County Health Department has received 2,500 doses
of H1N1 vaccine (nasal spray, pre-filled syringes and multi-dose
vials).
The Health Department has administered approximately 518 doses
of H1N1 vaccine to Ripley students/staff, EMS (Emergency Medical
Staff), licensed daycare providers, and article 28 clinic healthcare
workers.
The remaining doses of H1N1 vaccine have been reallocated to Pediatricians,
OB/GYNs, primary care providers and scheduled school-based clinics.
Public H1N1 clinic locations, dates and times have not yet been
determined due to the unknown volume of vaccine.

of November 2nd |
| 1. H1N1 vaccine was ordered last week
and Local Health Departments have received notification that they
will be receiving one fourth of the original order. The order should
arrive this week. This will be enough vaccine to continue to move
forward with the Silver Creek School-located H1N1 clinic.
2. The Silver Creek School Clinic dates are set for November 12th
& 13th starting at 9:00am.
3. Dunkirk School District dates have been tentatively set for
the week of November 16th. This is subject to change based upon
vaccine availability and school readiness.
Theses are the only dates tentatively scheduled.
4. Public H1N1 clinic locations, dates and times have not yet been
set due to the unknown volume of vaccine to be received by the Health
Department.
5. Local medical providers that have signed a provider agreement
with NYSDOH (New York State Department of Health) placed orders
last week (10/26/2009). Approximately 3,800 private physicians'
ordered doses will be shipped to the providers this week (11/02/2009).
Citizens are directed to check with their medical provider to determine
if they have signed an agreement and ordered vaccine.
*Note- 23 additional providers have been placed on H1N1 vaccine
shipment backorder*
6. Children 9 and under that have received their first dose of
H1N1 vaccine must receive a second dose 28 days or more after their
first dose. Second doses are to be received from their pediatrician,

| Ripley Central School District's
H1N1 clinic was conducted on Monday, October 26, 2009. 246 doses of
H1N1 vaccine were administered to the student population and staff
that meet the priority group criteria.
The Chautauqua Countyl Health Department placed the first H1N1
vaccine order on October 26, 2009. The order should arrive November
2, 2009. The amount of vaccine that will be received of the initial
order is unknown.
Public H1N1 clinic locations, dates and times have not yet been
determined due to the unknown volume of vaccine that will be received
November 2, 2009.
Silver Creek School District's H1N1 clinics have tentatively
been set for November 12 and 13, subject to change based upon vaccine
availability and school readiness.
Dunkirk School District's clinic dates have been tentatively
set for the week of November 16th.
These are the only dates tentatively scheduled.
Local providers that have signed a provider agreement with NYSDOH
can place H1N1 vaccine orders through the designated call center.

- A very limited supply (500 doses) of H1N1 flu mist was received
10/5/09.
- The doses have been offered to pediatrician practices.
- It was determined in counjunction with school superintendents
that:
* School located clinics are necessary during this public health
threat.
* School located clinics will be held ONLY for students &
staff that meet the priority population
criteria.
* School order was determined based upon a lottery system.
* Vaccine is currently in limited supply. School located clinic
dates will be determined as vaccine becomes more abundant.
* When school located clinic dates are set parents will be informed
directly by local school district.
* School districts will be responsibile for sending forms home
to parents prior to the set clinic date.
* The school based clinic schedule may be modified by County Health
Department based upon vaccine availability and school readiness.
- Large scale community clinics are not under consideration at
this time due to limited vaccine supply. School aged population
is the priority.
